Crestwood Medical Center recently announced that Brooke Uptagrafft, MD, was recently appointed to a three-year term to its Board of Trustees. Dr. Uptagrafft is a Family Medicine Physician and founder of Innova Primary Care, a team-based, advanced primary care practice and patient-centered medical home.
Dr. Uptagrafft earned her medical degree from the University of Alabama School of Medicine and completed her Family Medicine Residency at St. Vincent’s East where she served as Chief Resident. She founded Innova Primary Care in 2011 as a solo practitioner and has grown it to a 20-provider practice with locations in both Huntsville and Madison. She has served as the Chief of Medicine and Chief of Family Medicine at Crestwood Medical Center and currently serves on the Board of Directors for Blue Cross Blue Shield of Alabama. She also serves as a Medical Director for the Aledade Legacy Accountable Care Organization (ACO) guiding other practices across the state in quality and process improvement. She is also a certified nutrition coach through Precision Nutrition.
Dr. Uptagrafft and her husband, David, have two children, Claire and Jackson. In her off time, she enjoys spending time with her family, being outdoors and vacationing to Disney whenever possible.
